Community Alternative Care (CAC)
Crisis
Services that provide specific short-term care and intervention strategies to a person due to the need for relief and support of the care givers and/or protection of the person or others living with that person. This includes addressing both medical and behavioral needs.
Respite
Services provided to persons unable to care for themselves furnished on a short-term basis because of the absence or need for relief of those persons normally providing care.
PCA (Personal Care Attendant)
Service designed to support people of all ages with disabilities to live independently in the community. PCAs assist with daily living skills and health related functions in the person’s own environment. This is accomplished through hands on assistance, supervision, cuing, and redirection for behavior including observation and monitoring. PCA services are available to people of all ages with disabilities, people over 65 and people with chronic health conditions.
CTSS (Children’s Therapeutic Services and Supports)
CTSS is a flexible package of mental health services for children who require varying therapeutic and rehabilitative levls of intervention to address the conditions of emotional disturbance that impair and interfere with the individual’s ability to function independently. These services include Psychotherapy, skills training, crisis assistance, and Mental Health Behavioral Aide (MHBA) services.

Fourth Avenue Homes
Primarily serves consumers with long term mental health issues in Adult Foster care. Services also include a Children’s Respite and a home serving physically disabled.
Central Minnesota Senior Care
Offers Assisted Living Facilities in 11 location in the state for a comfortable, homelike setting, alternative to a nursing home. We also offer Intermittent Services and in Home Services.
