Service Team
Divine House, Inc. provides its consumers with a professional and diverse team deigned to meet the needs of the consumer.
Program Director
All Divine House Program Directors meet the state standards for QMRP or Designated Coordinator.
Program Directors work closely with Case Managers, Guardians and family to develop the treatment goals for each individual consumer. The Program Director provides continued oversight of the programming, providing written progress reports toward established goals to the consumer’s Interdisciplinary team.
Registered Nurse
Our RN’s are assigned specific sites. They are responsible for monitoring consumer health and medications. Divine House provides a “24/7 On Call” nursing pager system.
Program Coordinators
Program Coordinators are responsible to manage one home. They oversee the day to day care of consumers. This includes training direct service staff on the Individual Support Plan and Risk Management Plan of each consumer at the site and daily data collection toward established goals.
Human Service Technician
Human Service Technicians are the direct care staff who carry out the activities of daily living and goals associated with each consumer, recording progress and collecting data toward established goals.
Compliance Monitors
Monitors are assigned to a particular Program Director or division location to monitor all aspects of compliance with Rule 245B.
Quality Assurance
Quality Assurance (QA) monitors the overall quality of care provided for consumers and the quality of the physical sites. QA does internal investigations and tracks incident reports for the company. QA also takes new consumer referrals and works with counties for appropriate placement.
